I can provide you with a “Super Bill” at the end of the month that documents the charges, payment, and diagnosis when necessary for PPO insurance reimbursement.
My services are often covered by mental health PPO insurance policies, most likely your plan will cover therapy and you can see me as an “out-of-network” provider. I do not personally submit the paperwork to your PPO I will provide you with a super bill at the end of the month, which you can submit to your provider. A super bill is a standardized form that insurance companies recognize. The insurance company then issues a reimbursement check to you. It is up to your insurance company to determine if they will pay for services provided by a Licensed Marriage and Family Therapist. Patients are responsible for payment at the time of treatment.
You will have to check with your PPO to find out the type of coverage you carry and the limits of your coverage. To determine if you have mental health coverage, the first thing you should do is check with your insurance carrier.
Check your coverage carefully and find the answers to the following questions:
What are my mental health benefits? And what is the coverage amount per therapy session?
How many therapy sessions does my plan cover?
How much does my insurance pay for an out-of-network provider?
Is approval required from my primary care physician?
Payment is due at time of session.
For sliding scale clients must provide proof of income such as last year's W-2 and other documentation of your financial situation we can discuss an adjustment to your fee.
Cancellation Policy:
If it becomes necessary to cancel a scheduled appointment, please notify me as soon as possible. Please give at least 24-hour advance notice or you will be billed for the appointment. Notice can be given by telephone.
